Tourniquet Conversion

Tourniquet conversion is the act of removing a hasty tourniquet and replacing it with a pressure dressing (with or without hemostatic gauze) for wounds that do not require complete arterial occlusion for hemorrhage control. This procedure is done in the Tactical Field Care phase when immediate definitive treatment is not available. There are several different outcomes of tourniquet conversion attempts.

True Tourniquet conversion- This is when the wound is treated with hemostatic gauze and a pressure dressing. Tourniquet is then removed leaving only the dressing which adequately controls hemorrhage. This is ideal as it results in hemorrhage control of the wound with perfusion of the rest of the extremity. Goal is to do this in less than 2 hours from time of initial TQ application, don’t do it if TQ has been in place >6 hours.

Tourniquet Replacement- This is when a hasty tourniquet placed “high and tight” due to ongoing threat and need for rapid hemorrhage control is replaced with a distal tourniquet to limit iscmeia. The bleeding cannot be controlled with hemostatic and pressure dressing so the tourniquet is replaced with one that is 2-3 inches above the wound to allow perfusion of the limb above the wound. While this is not a true conversion, it does result in overall decreased limb ischemia.

Tourniquet to wide cuff. This is when a tactical or improvised tourniquet is exchanged with a wider tourniquet or cuff. This is useful in wounds that require a tourniquet for hemorrhage control and evacuation is delayed. The wider cuff results in less pressure force (pressure is spread out over a greater area) on the limb and can decrease tissue and nerve damage.

-완전히 동맥을 막지 않아도 출혈이 조절 되는 상처에 적용했던 지혈대를 압박 드레싱(지혈거즈가 없어도 가능함)으로 교체 하는 행위 이다
-즉각적인 치료를 못 할떄 TFC단계 에서 한다


완벽한 지혈대 전환
-지혈거즈와 압박 드레싱 으로만 으로 가능한 상황
-지혈대를 제거하고 출혈을 조절하는 드레싱만 남김
-사지에 제대로 피가 흐르면서 출혈이 조절 되기 때문에 제일 베스트다
-지혈대 적용시점 부터 2시간 이내에 적용하십쇼, 6시간 지났으면 안 됩니다


지혈대 교체
-초기에 빠르게 출혈을 막으려고 높은 곳에 적용한 경우 좀 더 상처에 가깝게 교체 하는 방법
-압력드레싱 으로 출혈을 조절 할 수 없는 상황이라 상처 2~3인치 위에 다시 적용한다
-완벽하지는 않지만 사지에 피가 순환이 덜 되는 것을 막는다


넓은 커프와 지혈대
-cat나 간이 지혈대를 더 넓은 지혈대나 커프로 바꾸는 것
-지혈대가 필요한 출혈+후송 까지 오래 걸리는 상처일때 적용함
-넓을수록 사지에 가해지는 압력이 줄어들어 조직과 신경등의 손상을 줄임
